If there is an accumulation (2 or more cases) of illness, please check the box below "two or more illnesses of similar severity" and enter the corresponding illness in the free text field.

There is no obligation to report individual cases of Covid-19, hand-mouth-foot disease, influenza or rubella. Please note that clusters must be reported.
