Draw up an assistance plan

Regardless of which form of "educational support" you apply for from the youth welfare office, the youth welfare office will draw up a support plan with you, your child if applicable, and other parties involved. In the help plan discussion, the appropriate help for the problem at hand and the measures to be taken are worked out.

The help plan sets out the key points of the help, such as

• Needs
  • Facts about your family (parenting situation, financial circumstances, parents' behavior, care of the child, physical and mental condition, etc.)
  • if you have already taken advantage of other support measures: What were they? Why was the help unsuccessful?
  • Your child's personal data sheet (e.g. who are the parents, siblings, relatives, abnormalities, illnesses, allergies, vaccinations)
• Aim of the support measures
  • What should change as a result of the help?
  • What are the expectations of those involved?
• Type of help
  • Which services are considered necessary and suitable (e.g. full-time care, parenting support, socio-educational family support)?
  • Suggestions for action for parents, professionals, caregivers
  • Time horizon (how long should the help last?)
  • Necessary services (e.g. care allowance, initial equipment, therapy costs, travel costs, financial support)

Note:The youth welfare office must inform all parties involved about the possible consequences of the planned help for the child or young person. It must also document in the assistance plan

• the way in which the parties involved work together and
• the extent of the advice given by the caregivers
